Professional Healthcare Resources, a leader in Home Health, Hospice and Personal Care services in the Washington, DC and Baltimore, Maryland area, seeks a passionate and enthusiastic RN Care Coordination Nurse  to join our team in Kensington location.
This is a salary based position!
 
POSITION SUMMARY
  • The RN Care Coordinator is a Registered Nurse who is responsible for promoting the delivery of coordinated, comprehensive care to patients in a designated Independent Living Facility.
  • Care coordination is accomplished through the activities of assessment, planning, documentation and provision of direct nursing care within the polices and procedures of the Agency.
  • RN Care Coordinator is responsible for conducting the Wellness Clinic in the facility at mutually agreed to hours between PHR and the facility.  In addition, the RN Care Coordinator assesses for the need for a different level of care such as home health or hospice services or need for a more comprehensive evaluation if a higher level of institutional care is needed and contacts the physician for further orders.  
  • The RN Care Coordinator is also responsible for completing home health admissions for referred patients within the facility and accurately completing the OASIS assessment using a point of care electronic documentation system, administering skilled nursing care to patients and determining what additional disciplines and/or programs are required for the care of the patient.
  • The RN Care Coordinator will also participate in staff education and delivery of training programs within an assigned geographic region.

The successful candidate must be a Registered Nurse with  at least 3 years’ experience in Home Health and OASIS
