Professional Healthcare Resources, a leader in Home Health, Hospice and Personal Care services in the Washington, DC and Baltimore, Maryland area, seeks a passionate and enthusiastic Admissions Coordinator to join our team in Annandale, VA.

Summary and Responsibilities:
A clinical professional responsible for collecting all information necessary to qualify and complete initial patient admissions and coordinate staffing through appropriate regional branch office; promotes quality services in accordance with established agency policies and procedures, assuring the quality and growth of the agency; complies with federal, state, and local guidelines for all services.

  1. Coordinates incoming referrals and facilitates staffing with agency offices to deliver services
  2. Communicates with community liaisons, other referrals sources, physicians, caregivers and families to gather required and necessary information
  3. Manages systems to capture and track incoming referrals; creates initial patient database; maintains patient intake records as required and necessary
  4. Records patient demographics, clinical diagnosis, clinical notes, physician contacts
  5. Confirms appropriate regional branch’s ability to staff and follow referrals
  6. Verifies physicians’ orders and licenses
  7. Communicates all required and necessary referral information, and supporting documentation, to appropriate regional/territorial team
  8. Schedules initial visits with appropriate staff/disciplines as required and necessary
  9. Communicates with branch staff in the implementation and documentation of agency policies and procedures that reflect quality and delivery of home health care services
  10. Communicates changes in intake operations and policies/procedures to branch offices in a timely manner
  11. Assists in disseminating standardized work processes to new and current employees
  12. Strengthens clinical quality and overall efficiency of delivery of home health care services
  13. Identifies clinical and non-clinical areas needing improvement; offers suggestions for system redesign
  14. Fulfills additional performance responsibilities as assigned and as necessary

Education and/or Experience
  • Must possess thorough knowledge of home health practices, procedures, and professional standards of care as set by Federal and state regulations, as well as Medicare regulations for home health services.
  • Three (3) years of home health care experience preferred
  • Previous experience of health agency intake processes preferred
  • Ability to read, analyze, and interpret technical procedures and governmental regulations
  • Proficient in reading and writing in English
