Professional Healthcare Resources, a leader in Home Health, Hospice and Personal Care services in the Washington, DC and Baltimore, Maryland area, seeks a passionate and enthusiastic Call Back Coordinator to join our team in Annandale, VA.


The Call Back Coordinator is a member of the clinical operations team who works under the supervision of the Director of Intake and Admissions performing duties to assist in the identification of potential health care needs of former patients. These duties will include, but are not limited to, running reports, completing follow-up phone calls, documentation of call findings and notification of appropriate individuals when new health care needs are identified. The Call Back Coordinator must communicate clearly and effectively with internal and external customers and make decisions using sound judgment, customer service principles and knowledge of home care requirements and Professional Healthcare Resources policies and procedures.


  • Requires one year of the home health experience.


  • Calls previous home care clients in accordance with established guidelines and risk stratification and frequency assignment.
  • Runs call back reports daily for assigned agencies in order to determine call sequencing. 
  • Provides complete, accurate and timely documentation regarding call back calls.
  • Completes all required reports.
  • Notifies appropriate agency personnel when a referral is identified via call back process and effectively communicates relevant information.
  • Completes follow up customer phone calls.
  • Demonstrates exceptional customer service skills in all phone calls and interactions.
  • Projects a professional image.
  • Assists in the review, tracking, and trending of call back program for all agencies.
  • Adheres to all privacy policies and maintains the confidentiality of protected health information (PHI).
  • Communicates requests and unresolved issues to the designated resource in a timely manner.
  • Provides administrative support to clinical operations when requested.
  • Other duties as assigned.
